All clients are assessed for high level care by the North West Regional Aged Care Assessment Team (ACAT) in Burnie.

The Community Packaged Care Programs available to care recipients from our service include.
Extended Aged Care at Home (EACH) program
This package is targeted to frail older people, aged 70 years and over or 50 years and over for Aboriginal and Torres Strait Islander people, with complex care needs who wish to remain living in their own home.
These people would otherwise be assessed as eligible for high level residential care.
Eligibility for a care package
To access a care package, a person must first be assessed as eligible by an Aged Care Assessment Team (ACAT).
A person will need to be assessed by an ACAT to decide if they are eligible to receive Australian Government funded care.

There are no citizenship or residency restrictions on accessing a Community Packaged Care Program. However, the programs are for ongoing care and should not be used for temporary care. This must be taken into account when considering eligibility.
The role of the ACAT
The ACAT will be responsible for the assessment of the care needs of prospective care recipients.

The ACAT provides information and help to access appropriate services according to eligibility criteria. The team may include health professionals such as a medical officer including geriatricians, nurse, social worker, physiotherapist, occupational therapist and psychologists.
The ACAT approves people for subsidised care, including residential care, CACP, EACH and EACHD packages and Transition Care. They also refer people to other appropriate care services.
Prospective care recipients may be referred to an ACAT from a wide range of sources including, but not limited to, carers, CACP, EACH and EACHD approved providers, Home and Community Care (HACC) providers, Veterans Home Care (VHC) or Veterans Community Nursing providers, general practitioners and health professionals; or they may self refer. If an assessment establishes eligibility, the ACAT will refer the potential care recipient to appropriate local approved providers.
